How is it possible for Stan O'Neal and his cohorts to have done so much damage?

O'Neal walks away with US$ 160m and the new CEO said that risk management was poor, too many silos, wrong incentive schemes etc, etc. These are direct criticisms of O'Neal. Why did they let him take all that money away?
